ABSTRACT

Objective To report a case of early pregnancy loss with septate uterus, successfully managed by hysteroscopy guided suction & evacuation of product of conception along with removal of uterine septum in same sitting. Case report. Tertiary care hospital. One woman with early pregnancy loss with septate Design Setting Patient uterus. Hysteroscopy guided suction & evacuation of product of conception along with removal of uterine septum Intervention in same sitting. Hysteroscopy guided suction & evacuation of product of conception followed by septum removal with Result hysteroscopic scissor was performed, with minimal blood loss, in same sitting. Products of conception were sent for pathologic examination. There were no intra-operative or postoperative complications. Even though the role of septum as a Conclusion contributing factor to miscarriage in not certain, early pregnancy loss may be seen in patients with septate uterus. Blind removal of POCs with dilatation & curettage, may be complicated with retained products of conception (RPOC) or intrauterine adhesion formation, which can lead to adverse fertility outcomes in the future. Also removal of septum, which may or may not be the cause, is often performed before further pregnancy. This requires another admission & exposure to anaesthesia, along with added cost. Hysteroscopy guided resection of POC with septum removal in same sitting has been associated with complete tissue removal under vision, less damage to surrounding endometrium, cost effective combination of two surgical procedures & earlier time to conception compared with 2 sitting procedure (dilation and curettage than septum removal). Thus, hysteroscopy guided suction & evacuation of POC & septum removal in same sitting can be considered as an alternative surgical technique for management of early pregnancy loss in patients diagnosed with septate uterus. This case report demonstrates the successful application of hysteroscopic procedure in a case of early pregnancy loss with septate uterus.

ABSTRACT

Resumo: A gestação é um fenômeno fisiológico e mesmo com o manejo de pré-natal adequado, as perdas gestacionais podem ocorrer. A Organização Mundial da Saúde estima uma ocorrência de 4,9 milhões de mortes perinatais no mundo todos os anos. No Brasil, a incidência em 2020, mostrou um total de 28.993 casos, sendo 1.062 no estado do Paraná e, destes, 117 na cidade de Curitiba. Não obstante, para além dos dados, tem-se o impacto emocional para todos os envolvidos, inquestionáveis para a mulher e família que sofre com a perda gestacional, mas também para os profissionais de saúde, em especial os enfermeiros, que se deparam com estas situações ao longo da vida profissional. Desta forma, este estudo teve como questão de pesquisa: qual a percepção do enfermeiro sobre o cuidado oferecido à mulher que sofreu perda gestacional? Ainda, como objetivo geral, compreender a percepção do enfermeiro que presta cuidados às mulheres diante das perdas gestacionais; e objetivos específicos: identificar elementos do cuidado do enfermeiro frente à mulher que sofreu perda gestacional e descrever a experiência dos enfermeiros sobre seu cuidado diante da mulher que diante perda gestacional. Trata-se de estudo qualitativo exploratório, realizado em um hospital universitário da Região Sul do país. A coleta de dados deu-se através de entrevistas semiestruturadas, as quais foram audiogravadas e transcritas na íntegra, durante os meses de abril a junho de 2022. Contou com a participação de 11 enfermeiros que atuam diretamente com mulheres em situação de perda gestacional. Para análise dos dados, foram seguidos os passos da Análise de Conteúdo do tipo temática proposta por Bardin e apoiada pelo uso do software de análise qualitativa o webQDA. Resultados: foram levantados 13 temas, os quais, por afinidade e exclusão, resultaram na elaboração de 3 categorias: Fragilidades do processo de cuidar frente às perdas gestacionais; Potencialidades do processo de cuidar frente às perdas gestacionais; e Cuidados de enfermagem frente às perdas gestacionais. Foi possível evidenciar que os enfermeiros conseguiam reconhecer as lacunas assistenciais e, mesmo com suas dificuldades, prestavam um cuidado respeitoso e empático, além de proporcionar momento de criação de memórias para as famílias enlutadas, através de fotos ou guarda de pertences que foram do bebê, que passou tão brevemente pela vida. O estudo permitiu compreender que os enfermeiros entrevistados reconheciam a importância da sua presença como um profissional que oferece um cuidado direcionado a cada mulher/família em situação de perda gestacional nas suas necessidades, enfatizaram a comunicação verbal e não verbal, sendo uma de suas preocupações que esta compreendesse, para além da assistência oferecida, este olhar singular do profissional. Os impactos do desenvolvimento deste trabalho podem resultar na elaboração de documentos, materiais educativos e protocolos assistenciais voltados aos profissionais de saúde que prestam cuidados diante das perdas gestacionais dentro das instituições hospitalares.


Abstract: Pregnancy is a physiological phenomenon. However, even with proper prenatal care management, pregnancy losses may occur. The World Health Organization estimates 4.9 million perinatal deaths worldwide every year. In Brazil, showed an incidence of 28,993 cases in 2020, with 1,062 cases in Parana State and, among these, 117 in its capital city, Curitiba. Therefore, that is a global health problem, but there still have been scarce public policies addressing this theme. Apart from the data, there is also the emotional impact on all the involved individuals, unquestionably to the woman and family who suffer the pregnancy loss, but also to the healthcare professionals, ultimately nurses, who are confronted with those situations along their professional lives. Thus, the research question in this study was: what is the nurses' perception on the health care delivered to the woman facing a pregnancy loss? In addition, the general objective was to understand the nurse's perception who renders health care to the woman who suffered a pregnancy loss. It is an exploratory qualitative study held at a university hospital in Southern Brazil. Data collection was conducted by means of semi-structured interviews, which were audio recorded and fully transcribed between April and June 2022. The participants were 11 nurses who care for women in a situation of pregnancy loss. For the data analysis, Thematic Content Analysis proposed by Bardin was applied, supported by the use of the webQDAE software for qualitative analysis. Results: 13 themes by affinity and exclusion stood out by means of the analysis, which resulted in the elaboration of 3 categories: Fragilities in the caring process in the face of pregnancy losses; Potentialities of the caring process in the face of pregnancy losses; and Nursing Care in the face of pregnancy losses. It was possible to evidence that nurses could recognize caring gaps, and even facing difficulties, they could deliver respectful and empathetic care, in addition to providing a moment of creating memories to bereaved families through photos or belongings of the baby who briefly got through their lives. The study enabled to understand that the interviewed nurses acknowledged the importance of their presence as professionals who delivered care to meet the needs of each woman/family going through a pregnancy loss. They also pointed out verbal and non-verbal communication as one of their concerns so that the women could understand their unique professional look, beyond care delivery. The study outcomes may result in the elaboration of documents, educational materials and care protocols to those health professionals who are confronted with pregnancy losses while rendering care within hospital institutions.

Abstract Objective Recurrent miscarriage has been linked to hormonal disturbance due to dysregulation of its receptors rather than to the availability of the hormone. We aimed to investigate endometrial expression of progesterone and estrogen receptors in relation to serum and endometrial hormonal levels in unexplained recurrent miscarriage. Methods The present case control study included 20 cases with unexplained recurrent miscarriage and 20 parous women as controls. Ovulation was confirmed using an ovulation kit and 10 to 12 days after detecting the urinary luteinizing hormone surge, all women were subjected to a blood sample and to an endometrial biopsy. Progesterone and estrogen levels were measured in serum and in endometrial tissue and receptor concentrations were in the endometrial sample. Results Women with recurrent miscarriage showed significantly lower concentration of receptors in both the cytoplasm and the nucleus of endometrial tissue compared with controls. The nuclear/cytoplasm ratio of progesterone receptor was significantly higher in cases compared with controls, implicating that recurrent miscarriage is probably linked to nongenomic activity of the hormone; this was also significant for estrogen receptor. Serum progesterone and estrogen hormonal levels were comparable between groups while both hormones were significantly reduced in the endometrium of recurrent miscarriage cases. Receptors significantly correlated with endometrial hormonal level but not to serum level. Conclusion Recurrent miscarriage might be linked to reduced endometrial progesterone and estrogen receptors and appears to be more related to nongenomic activity of progesterone. Endometrial receptors expression correlates to tissue hormonal level rather than to serum hormonal level.

ABSTRACT

Uterine malformations can result from various alterations in the normal development of Mullerian ducts that make up a diverse category of congenital anomalies. In most cases, the defect remains undiagnosed and exhibits the normal number of chromosomes. Very few malformations require any intervention after thorough investigations. The most frequently used classification by the American Society for Reproductive Medicine is comprehensive, but does not include cervical or vaginal malformations. The European Society of Human Reproduction and Embryology has postulated a consensus that precisely shows independent cervical malformations.

ABSTRACT

Pasteurella pneumotropica is a bacterium that has so far not been described as a cause of placentitis in animals. Two cases of aborted equine fetuses were sent to the Department of Veterinary Pathology of the "Universidade Federal do Rio Grande do Sul" (SPV-UFRGS) for anatomopathological examination. Both cases presented suppurative placentitis associated with multiple basophilic bacterial cells. After bacterial isolation and biochemical analysis, P. pneumotropica was identified.(AU)

Resumen OBJETIVO: Determinar la prevalencia de los principales factores etiológicos de pérdida gestacional recurrente en la población de un hospital de tercer nivel de atención. MATERIALES Y MÉTODOS: Estudio retrospectivo, transversal y descriptivo llevado a cabo en pacientes con protocolo de estudio de dos o más pérdidas gestacionales recurrentes que iniciaron el control prenatal en el servicio de Obstetricia o que ingresaron al Instituto Nacional de Perinatología con diagnóstico de infertilidad entre los meses de enero de 2017 a enero de 2020. En cada grupo se revisaron los factores etiológicos descritos en la bibliografía internacional como posibles causas de pérdida gestacional recurrente. RESULTADOS: Se estudiaron 280 pacientes y el factor con mayor prevalencia de pérdida gestacional recurrente fue el endocrino con el 56.78% (n = 159), seguido del anatómico-uterino con el 42.14% (n = 118) y en tercer lugar el infeccioso con 40.35% (n = 113). En las 75 pacientes del grupo de infertilidad, el factor etiológico más prevalente fue el endocrino (88%; n = 66), seguido del anatómico-uterino (53.3%; n = 40) y a continuación del masculino (50.6%; n = 38). En las pacientes del grupo de Obstetricia, el factor con mayor prevalencia fue el endocrino (45.36%; n = 93), seguido del anatómico-uterino (38.04%; n = 78) y el infeccioso (37%; n = 76). CONCLUSIONES: Los factores relacionados con la pérdida gestacional recurrente, descritos en la bibliografía internacional y analizados en este estudio, mostraron prevalencias similares en población institucional. Las que difieren están influidas por las características de la población estudiada, los recursos de la institución e, incluso, el diagnóstico y servicio por el que ingresaron como pacientes al INPer.


Abstract OBJECTIVE: To determine the prevalence of the main etiological factors of recurrent gestational loss in the population of a tertiary care hospital. MATERIALS AND METHODS: Retrospective, cross-sectional, descriptive study carried out in patients with study protocol of two or more recurrent gestational losses who initiated prenatal control in the Obstetrics service or who were admitted to the Instituto Nacional de Perinatologia with a diagnosis of infertility between the months of January 2017 to January 2020. In each group, the etiological factors described in the international literature as possible causes of recurrent gestational loss were reviewed. RESULTS: 280 patients were studied and the factor with the highest prevalence of recurrent gestational loss was endocrine with 56.78% (n = 159), followed by anatomic-uterine with 42.14% (n = 118) and thirdly infectious with 40.35% (n = 113). Abstract Objective The aim of the present study was to examine the relation between the PON1 polymorphisms and recurrent pregnancy loss (RPL). Methods In a cross-sectional study, blood samples were collected from 100 females. DNA was extracted and PON1 genotypes were determined by polymerase chain reaction (PCR) amplification. Results Regarding PON1 L55M, the mutated allele (M) frequency was found in 70.5% in RPL and in 53.5% in controls; theMallele was significantly associated with an increased risk of RPL (adjusted odds ratio [ORadj]=2.07; 95% confidence interval [CI]; p<0.001). However, regarding PON1 Q192R, the R mutated allele frequency was found in 28.5% in RPL and in 33% in controls. The R allele did not show any risk for RPL (ORadj 0.81; 95%CI; p=0.329). Conclusion The present study suggests that there is an effect of genetic polymorphism on RPL and provides additional evidence that combines with the growing information about the ways in which certain PON1 genotypes can affect the development of the fetus in the uterus.


Resumo Objetivo O objetivo deste estudo foi examinar a relação entre os polimorfismos PON1 e perda recorrente de gravidez PRG. Métodos Em um estudo transversal, foramcoletadas amostras de sangue de 100 mulheres. O DNA foi extraído e os genótipos PON1 foram determinados por amplificação por PCR. Resultados Com relação ao PON1 L55M, a frequência do alelo mutado (M) foi encontrada em 70,5% no PRG e em 53,5% nos controles; o alelo M foi significativamente associado a um risco aumentado de PRG (odds radio ajustado [ORadj] =2,07; intervalo de confiança [IC] 95%; p<0,001). No entanto, em relação ao PON1 Q192R, a frequência do alelo mutado R foi encontrada em 28,5% no PRG e em 33% nos controles. O alelo R não mostrou qualquer risco para PRG (ORadj 0,81; IC 95; p=0,329). Conclusão O presente estudo sugere que há um efeito do polimorfismo genético sobre PRG e fornece evidências adicionais que se combinam com as informações crescentes sobre asmaneiras pelas quais certos genótipos PON1 podemafetar o desenvolvimento do feto no útero.

Resumen: Las pérdidas de embarazos son una complicación obstétrica frecuente. Se conoce que, 15% de las mujeres embarazadas tienen al menos una pérdida esporádica. El 5% experimentan 2 pérdidas, y hasta un 1% 3 o más. La edad materna avanzada, la multiparidad y el antecedente de pérdida de embarazo previo aumentan el riesgo. La vinculación de los estados protrombóticos, hereditarios y adquiridos, con las pérdidas de embarazo se relaciona con el hecho de que para que se mantenga el embarazo es necesario exista una adecuada circulación placentaria. A lo largo de los años se ha estudiado la relación que existe entre los diferentes estados protrombóticos hereditarios y adquiridos con estas complicaciones vasculares que determinan pérdidas de embarazo y complicaciones obstétricas. Se realiza una revisión sobre las trombofilias hereditarias y adquiridas con ésta entidad.


Abstract: Pregnancy losses are a common obstetric complication. It is known that 15% of pregnant women have at least one sporadic loss. 5% experience 2 losses, and up to 1% 3 or more. Advanced maternal age, multiparity, and a history of prior pregnancy loss increase the risk. The link between hereditary and acquired prothrombotic states with pregnancy losses is related to the fact that adequate placental circulation is necessary for the pregnancy to be maintained. Over the years, the relationship between the different hereditary and acquired prothrombotic states with these vascular complications that lead to pregnancy losses and obstetric complications has been studied. A review is carried out on the hereditary and acquired thrombophilias with this entity.


Resumo: A perda da gravidez é uma complicação obstétrica comum. Sabe-se que 15% das gestantes apresentam pelo menos uma perda esporádica. 5% experimentam 2 perdas e até 1% 3 ou mais. Idade materna avançada, multiparidade e história de perda de gravidez anterior aumentam o risco. A ligação entre os estados protrombóticos hereditários e adquiridos com as perdas gestacionais está relacionada ao fato de que a circulação placentária adequada é necessária para a manutenção da gravidez. Ao longo dos anos, estudou-se a relação entre os diferentes estados pró-trombóticos hereditários e adquiridos com essas complicações vasculares que levam à perda da gravidez e complicações obstétricas. É realizada uma revisão das trombofilias hereditárias e adquiridas com essa entidade.

ABSTRACT

Background: Recurrent pregnancy loss is physically and emotionally harmful for mothers. This study aimed to find out the prevalence and associated factors of recurrent pregnancy loss (RPL) in pregnant women in Duhok province.Methods: In this retrospective study, the medical records of the patients who had pregnancy loss and registered in Duhok Maternity Hospital were reviewed for the period of January - December 2019. Accordingly, 300 women aged 18 years and older with pregnancy loss were included in this study.Results: The prevalence of EPL was 16.3%. The study found that patients with RPL were statistically older compared to those patients without RPL, (39.27 vs. 33.24 years; P<0.001), respectively. Besides, the RPL group had a significantly higher percentage of family history of pregnancy loss (12.24% vs. 1.20%; P<0.001). The patients with RPL had significantly higher prevalence of Chlamydia trachomatis (14.29% vs. 3.59%; P=0.002), cervical incompetence (8.16% vs. 0.80%; P=0.002), structural abnormalities of uterus (14.29% vs. 4.8%; P=0.012), and polycystic ovary syndrome (PCOS) (40.82% vs. 11.95%; P<0.001). There was no significant association of RPL with smoking (P=0.261).Conclusions: This study revealed a high prevalence of RPL in patients with pregnancy loss. Also, the study showed that the patients in the RPL group were significantly older and had a higher prevalence of medical illnesses; including chlamydia trachomatis, cervical incompetence, structural anomalies of the uterus, and polycystic ovarian syndrome.

ABSTRACT

Congenital uterine anomalies occur due to abnormal fusion of mullerian duct during embryonic life. It is associated with high incidences of reproductive failures and adverse obstetrical outcomes. It may be associated with malpresentation, preterm labour, or recurrent pregnancy losses. Authors report a case series of 7 patients which were admitted in obstetrics and gynaecology department of SP medical college and associated group of hospitals between time period of March 2019 to July 2019. Among 7 cases 6 cases were associated with malpresentation, 1 with abortion, and 1 with preterm labour. This series shows that uterine anomalies are associated with different obstetrical outcomes varying from totally uneventful antenatal and postnatal period to abortion or preterm labour. Prenatal diagnosis of uterine anomaly may help in improving the obstetrical outcome in these patients.

ABSTRACT

The pregnancy is an immunocompromised state. Thus, autoimmune diseases may affect pregnancy and get worsen during pregnancy. Here authors discuss a rare autoimmune thrombophilia disorder, protein C and S deficiency which may cause recurrent pregnancy losses by affecting haemostatic mechanisms in the body. This patient with recurrent pregnancy loss when evaluated extensively was found to have combined inherited protein C and S deficiency. It was successfully managed with thromboprophylaxis therapy, which resulted in the delivery of healthy baby. Long term anticoagulant prophylaxis should be considered weighing the risk of bleeding to thrombotic recurrence in such cases. In conclusion, combined protein C and S deficiency and that too presenting as recurrent pregnancy loss is very rare. Thrombophilia screening should be considered in cases of recurrent pregnancy losses. Adequate and appropriate thromboprophylaxis is an important part of the management of pregnant women with inherited thrombophilia.

ABSTRACT

Anomalies of the reproductive tract are common and are seen in approximately 3-5% of the general population. These are usually asymptomatic, but are sometimes associated with recurrent pregnancy loss or infertility. Among these anomalies, septate uterus is the most common anomaly to be associated with obstetric complications and infertility. A 27 years old female patient P0A2L0 with first spontaneous abortion at 3 months and second spontaneous abortion at 4 months came to OPD with complaint of recurrent pregnancy loss. She was advised USG of pelvic region which shows septate uterus with normal cervix, fallopian tubes and ovaries. Patient prepared for DHL complete septum present normal B/L ostium and Jone's metroplasty done in which wedge shaped incision kept on uterine fundus, common cavity created by un roofing septum myometrium closed and AT the end Copper T 375 placed in intrauterine cavity for 6 months. 1 year after surgery patient came with UPT positive for antenatal visit at sola civil. Patient is now with 30 weeks of pregnancy with normal single intrauterine live cephalic fetus, metroplasty is an accepted method of treatment in women with recurrent abortions and septate uterus and it significantly improves the subsequent reproductive outcome. combined data from several published series and reported that the incidence of spontaneous abortion and preterm delivery rate decreases significantly after metroplasty, whereas, the incidence of term delivery rate increases.

Background: Recurrent pregnancy loss (RPL) is a serious problem on the women, it defined as two or more consecutive pregnancy losses before the fetus has reached birth. The aim of this study is to evaluate the association between the elevation in the factor VIII and RPL. Because women who have thrombophilia have increased risk of fetal loss in most studies.Methods: A total 72 women were recruited in this case control study. They divided into two groups: the RPL group included 41 women with a history of recurrent pregnancy loss and the control group included 31 healthy women, who had at least one successful pregnancy and none of them had a history of fetal loss or complicated pregnancy.Results: A majority of the patients of this study didn't have a high level of factor VIII, 9 of 41 (22%) patients of RPL group in comparison with 21 of 32 (65,6%) of control group, that suffer from the increase rate of FVIII, this means that factor VIII doesn't effect on RPL.Conclusions: The present study showed that the serum elevation in the factor VIII is not significantly associated with RPL.

ABSTRACT

Background: Approximately 1-3% of women of reproductive age suffer from recurrent pregnancy loss. Objective of this study was to evaluate the association between recurrent pregnancy loss and thrombophilia.Methods: This is a descriptive study, involving retrospective analysis of patients with recurrent pregnancy losses. Patients with recurrent pregnancy loss in whom associated morbidity factors were excluded underwent screening for both acquired and inherited thrombophilia.Results: A total of 20 patients were screened for acquired and inherited thrombophilia with recurrent pregnancy loss. Thrombophilia was diagnosed in 70% cases. Out of which, anticardiolipin antibodies was found positive in 57% of patients, protein C 7% and protein S deficiency was observed in 35% cases.Conclusions: Thrombophilias are associated with recurrent pregnancy loss. Patients in whom other associated morbid factors are excluded, should be offered screening for thrombophilia. Multidisciplinary management involving hematologist is vital for management.

ABSTRACT

Background: To compare the role of low dose aspirin versus combination of low dose aspirin and low molecular weight heparin in idiopathic recurrent pregnancy loss and assess the effectiveness of low dose aspirin and low molecular weight heparin in having a better obstetric outcome.Methods: This study was conducted in a private hospital in Mahabubnagar from June 2017 to May 2019. A total of 80 pregnant ladies who had previous 2 and or more pregnancy losses in the early (before 20 weeks) or late (after 20 weeks) pregnancy period was included in the study. 80 pregnant women with idiopathic/unexplained recurrent pregnancy loss were properly evaluated in regard to the history of previous period of gestation of loss, previous scans in regard to documentation of fetal pole and gestation, cardiac activity, anomaly scan and growth scan and any special investigations in previous pregnancies and present pregnancy.Results: A total 80 pregnant women with previous 2 and more unexplained pregnancy losses who were evaluated and found negative with major causes of pregnancy losses half of them (40) were treated with low dose aspirin alone and the other 40 women were treated with a combination of low dose aspirin (75 mg) and low molecular weight Heparin (20 mg) daily low molecular weight heparin till term. The aspirin alone group had 82.5% live birth rate and the combination group had 92.5% live birth rate which is quite satisfactory and more than the Aspirin alone group.Conclusions: Use of combination of low dose aspirin and low molecular weight heparin seems to be a good choice of drugs in treating the unexplained recurrent pregnancy losses than low dose aspirin alone.

ABSTRACT

Objective: To establish and verity predictive model of pregnancy loss in systemic lupus erythematosus (SLE). Methods: A total of 338 SLE pregnant patients admitted to Renji Hospital, Shanghai Jiao Tong University School of Medicine from Sept. 2011 to May 2017 (model development group) and 131 SLE pregnant patients admitted from Jun. 2017 to Jun. 2018 (model validation group) were selected. Multivariable Logistic regression model was used to determine the predictive variables and their coefficients of pregnancy loss in model development group. The predictive model was established, the risk score classification was performed, and model validation group was used for external validation. Results: Multivariate Logistic regression analysis showed that unplanned pregnancy (P=0.032), low complement C3 (P=0.002) and 24 h urinary protein ≥ 1.0 g (P=0.000) were the risk factors of the predictive model of SLE pregnancy loss. When the risk score of the model was 0-3, the risk of SLE pregnancy loss was low, and when the risk score is more than 3, it is high risk, with a sensitivity and specificity of 60.5% and 93.3%, respectively. The model was used in the model validation group for external validation, and the prediction accuracy of SLE pregnancy loss was 90.1%. Conclusion: The predictive model of SLE pregnancy loss can help clinicians efficiently screen the high-risk population of SLE pregnancy loss in order to take relevant measures as soon as possible to obtain better pregnancy outcomes.

We aimed to study the association between sperm DNA fragmentation and recurrent pregnancy loss (RPL) in the Chinese population via a retrospective observational study of Chinese couples who had experienced RPL between May 2013 and August 2018. The study population included 461 men from couples with RPL and 411 men from a control group (couples with clinical pregnancy via in vitro fertilization owing to female causes). Routine semen analysis, sperm chromatin analysis, and microscopic (high-power) morphological analysis were performed using semen samples. Semen samples were assessed for volume, sperm count, and motility. The sperm DNA fragmentation index (DFI) was calculated, and the median DFI was obtained. Men were categorized as having normal (37.8%; DFI ≤ 15.0%), moderate (33.6%; 15.0% < DFI < 30.0%), or severe (28.6%; DFI ≥ 30.0%) DNA fragmentation levels. The percentage of men with severe DNA fragmentation was significantly higher in the RPL (42.3%) group than that in the control group (13.1%), whereas the percentage of men with normal levels of DNA fragmentation was significantly lower in the RPL group (22.8%) than that in the control group (54.7%). Subsequent analysis also demonstrated that the sperm DNA fragmentation rate had a moderate reverse correlation with the sperm progressive motility rate (r = -0.47, P < 0.001) and the total motile sperm count (r = -0.31, P < 0.001). We found a positive correlation between RPL and sperm DNA fragmentation. The results suggest that increased sperm DNA damage is associated with RPL.

ABSTRACT Introduction: Although there is a vast literature regarding the association between inherited thrombophilia, obstetric complications and the effect of low molecular weight heparin (LMWH), these are controversial and we have not found publications related to additional risk factors other than thrombophilia.Our objectives were to assess the prevalence of miscarriage, placenta-mediated pregnancy complications and fetal loss in pregnant women with IT, establishing associated risk factors and the effect of LMWH. Materials and methods: A retrospective cohort of pregnant women with IT was formed. Risk factors considered were: high-risk IT, age ≥35 years, body mass index ≥25 and ≥30, assisted reproductive technology, antiphospholipid antibodies, autoimmune disease, first-degree family history of obstetric complications and personal history of venous or arterial thromboembolic disease, the outcomes being M, FL and PMPC. Results and conclusions: Data from 250 pregnancies in 88 women were obtained.There were 112 (45%) Ms, 13 (5.2%) FLs and 25 (10%) PMPCs.High-risk IT was associated with FL (OR = 4.96; 95% CI, 1.42-17.3). Antiphospholipid antibodies and family history of obstetric complications were associated with PMPC (OR = 7.12; 95% CI, 1.89-26.74, OR = 3.88; 95% CI, 1.18-12.78, respectively). The LMWH presented a benefit in the combined outcome (any obstetric complication, OR = 0.25; 95% CI, 0.12-0.54) and M (OR = 0.41; 95% CI, 0.20-0.82).We conclude that obstetric complications are common in women with IT. Antiphospholipid antibodies, family history of obstetric complications and high-risk IT might be additional risk factors. The LMWH has an apparent protective effect against obstetric complications, which is consistent with some previous studies.
